I had a 95 LS and the tilt would not move the wheel.Had to take column apart and use a hose washer to sync the gear up to the teeth.It just needed that extra length to be able to engage.Easy job.Doubt it took more than 10 minutes.Plenty of Lexus forums out there with info.Great cars.Good Luck sir

Is the battery topped off and in good condition, LS 400 series is notorious for battery draw, if it is two low, secondary systems can fail, steering wheel tilt and power seat adjustment. There should be a screw under the column holding the plastic surround on the steering column.

The Gears could have seized in place due to lack of lubrication OR it is a motor failure that actuates the up, down, in, out (That's what she said) OR an electrical short.

I found a Youtube video that shows how easy it is to open it up to figure it out from a Lexus Forum I just looked up.. Stand by to stand by:

I'm a huge car nut and a huge luxury car nut, and I've owned all except a Lexus LS…it will indeed be my next car…these cars are phenomenal and will out last any German high end luxury car, and this is coming from a current Audi owner. These high end Toyota cars will run well over 500k dam near close to a million miles without engine work. No joke!

I fixed this myself on my 92 ls400 I honestly was bored and said fuck it, watched a YouTube clip and used zip ties cuz I couldn't find rubber washers, they make a replacement gear as well but I didn't feel like waiting for shipping and was more than satisfied with the outcome…

Take off the cover around the column. Its either going to be the plastic gear or the motor Both on left side.
