That M54 sounds pretty bad.
It’s the maf sensor, once I had an e46 auto and I was working on it and forgot to connect maf sensor and when I drove it, it indicated the gear light on my dash and was driving poorly shifting like garbage. Fix the MAF sensor first. !!!

I have a 2002 530i almost exactly like this, but mine has the sport package with lower suspension and other goodies from the M5. It has 178k miles, and still drives like new. The E39 530i is a hidden gem. They are amazing cars.
Transmission is most likely fine. Common issue is the gear selector valve in the transmission. It plastic and can snap. Then the transmission will most likley be stuck in reverse and all the other gears will be neutrals or reverse since the gear selector isent connected inside in the valve body no more. It will often snap when selecting into reverse.
Is Brady's watch an old Fossil? Brought me back to middle school seeing that one.

Loved my 525i 2001 with a 5 speed stick was a fantastic car sold it for 700 with 235K miles.

looks like someone robbed the MAF and swapped it with a different one from a different year, thats probably why that connector wont fit on the MAF.
