It looks to me like they felt their way for that bolt, but missed. However they thought they found the hole and started screwing the bolt. They may have thought it was tight due to the tension not visually checking the bolt. Just my assumption.

I went through this with my 3.9. Loss power backfire. Stalling. Coil is dying. Not an engine or fuel issue. Changed my coil right as rain. Of course my 3.9 was carb so…. But same symptoms exactly

I would suggest that a radiator flush be run on the engine if it passes a compression and leakdown test.

To me like they were feeling down there for the bolt hole felt it catch and threaded it in there rather than the actual hole where it goes
