If you remove it and shake it up and down you should here a ball moving around in it. if not it may be time to replace it.

how would you know if your pcv valve is bad?

Pull out teh valve and grommet with needle nose pliers being careful not to let any pieces fall in to the engine. I don’t know that this is a problem on these 6.0L. This truck is 10 years old and has 273,000 miles and it came apart and went back together with no issue.

Normally if the rubber has dried up, you will have to replace that at the same time that way you won’t end up with a vacuum leak.

What do you do when the grommet that the PCV fits through gets hard and inflexible? I’ve had a few that seem impossible to pull out because of that.
